HOURS & DAYS OF OPERATION
The Center is open Monday – Friday from 7:30AM to 5:30PM
Adult Day Care
| Basic Day Care rate which includes two meals & a snack. | $65 per day |

Applicants must be: • Ambulatory and independent (with or without an assistive device) • Continent • Able to feed themselves • A non-wanderer (individual who does not endanger themselves by consistently trying to leave the premises • Non-combative |

| Add-on (surcharged) services, not included in the Adult Day Care rates, and charged each day of delivery unless otherwise indicated: | |
| 1. Extended Time | |
| • Early Drop Off (between 7:00am and 7:15am) | $30 flat rate |
| • Early Drop Off (between 7:16am and 7:30am) | $15 flat rate |
| • Late pickup (between 5:30pm and 5:45pm) | $15 flat rate |
| • Late pickup (between 5:46pm and 6:00pm) | $30 flat rate |
| 2. Third Meal of the day | $5.25 |
| 3. Mechanically altered meals | $5 per day |
| 4. Shower | $21 |
| 5. Extended Assistance Program | $15 per day |

Inclues: • Proactive Toilet Program • Redirection/Engagement Program • Assistance with feeding |

| 7. Added special needs are to be priced by the Program Director | |
| • Incontinence assistance currently $18 per incident | |
| 8. Excursion costs will be determined at the time of the excursion | |
ADDITIONAL INFORMATION
Special Equipment. If a client requires a wheelchair, walker or cane, these items must be provided by the caregiver/family.
Change of clothing. Clients should have an extra change of clothes, including underwear, to be kept at the center. Please label them.
Geriatric Pads/Briefs. Clients using geriatric pads/briefs should provide ample supply to be kept at the center for their use.
