Persons interested in home health services should inquire by calling
(808) 440-3041 during office hours Monday-Friday, 8:00 a.m. - 5:00 p.m.
The applicant and/or family will be fully informed about the array of services available, eligibility, and costs. Personal appointments can be accommodated by calling the Director at (808) 440-3038 for an in-home visit or a meeting at the AHHS office, located at 1660 S. Beretania Street, Room 203, Honolulu, HI 96826.
For Personal Care, Limited Health Care, Assistance with Transportation/Escort Services, or Home Spa services, the following forms must be submitted:
• Each applicant is required to have a physical examination by alicensed physician prior to initiating home health services
• Following submission of the forms above, an appointment for an initial assessment is scheduled with the Registered Nurse and Director. Family members or caregivers may also be present.
• After the assessment, a Service Plan is developed based of the needs of the applicant and input by the family/caregivers, the Registered Nurse, and the Director.

• On the first scheduled visit for Homemaker Services, the Director will meet with the applicant and family members to develop the Service Plan.

• Initial intake information is provided to us over the phone
• Order meals with a minimum of 24 hours notice
• Cancel any meal orders with a minimum of 24 hours notice
