APPLICATION PROCEDURE

Persons interested in home health services should inquire by calling
(808) 983-5900 during office hours Monday-Friday, 8:00 a.m. - 4:00 p.m.

The applicant and/or family will be informed about the array of services available, eligibility, and costs. Personal appointments can be accommodated by calling (808) 983-5900 for an in-home visit or a meeting at the AHHS office, located at 1660 S. Beretania Street, Suite 201, Honolulu, HI 96826.

For Personal Care, Limited Health Care, Assistance with Transportation/Escort Services, or Home Spa Services, the following forms must be submitted:

Emergency Information form

History and Physical form

 

• Each applicant is required to have a physical examination by a licensed physician prior to initiating home health services
• Following submission of the forms above, an appointment for an initial assessment is scheduled. Family members or caregivers may also be present.
• After the assessment, a Service Plan is developed based on the needs of the applicant and input by the family/caregivers and AHHS.

HOUSEKEEPING SERVICES

Persons interested in housekeeping services should inquire by calling
(808) 983-5900 during office hours.

The following forms must be submitted before the home evaluation:

• Following the home evaluation, cleaning visits are scheduled.

HOME DELIVERED MEALS

Please call (808) 983-5900 for information about this program. Your name can be added to our mailing list so that you receive our monthly menus for meal planning.

• Initial intake information is provided to us over the phone
• Order meals with a minimum of 24 hours notice
• Cancel any meal orders with a minimum of 24 hours notice
